Abstract: The Parent Information Center (PIC) in New Hampshire serves as a vital statewide resource center for families of youth with disabilities or special healthcare needs, as well as the professionals who support them. PIC's mission revolves around empowering families and individuals to make informed decisions that promote development and well-being. Central to their efforts is the Life After High School Online Learning Module, designed to assist caregivers in guiding their youth through the transition process. This abstract outlines the collaborative efforts undertaken to create caregiver modules, with a focus on incorporating youth perspectives and emotional components into transition planning. The involvement of the YEAH Council, a youth-driven group advocating for individuals with disabilities, underscores the importance of placing youth at the forefront of secondary transition planning. The caregiver modules emphasize tools for empowerment, self-determination, and fostering emotional wellness in both caregivers and youth. Future directions include the publication of these modules on the PIC website, ongoing collaboration with mental health professionals, and the continued development of transition-related resources in partnership with the YEAH Council.
